Please note: for Sunday pickup, you will be charged a full day boarding fee.

By agreeing to the terms of NAH Sunday Boarding pickup I understand that should I need to contact NAH on Sunday I may only do so between 4:00pm and 5:00pm during the pickup time by calling 203-957-0829. All pickup will occur at the back door as instructed by the attending kennel staff.
I authorize Norwalk Animal Hospital (NAH) to charge the credit card provided below for all services rendered during the patient(s) stay. I understand the card will be charged the full amount due at time of pickup. I understand that should any additional services be requested or provided to my pet(s), such as vaccinations, emergency treatment or any other services or treatments that may be necessary, the credit card provided below will be charged additionally based on requested services or selection on the back of the signed yellow boarding cage card.(Required)
